If youre interested
in the possibility of being partnered with one of our
remarkable Service Dog, please contact us for an information
packet.
Upon reading the information packet, if you decide that
you would like to apply , heres what to expect.
A completed application includes the following:
- A $25 application
fee.
- The completed Program
Application form.
- The Medical History
form completed by your physician or primary care specialist.
- A personal letter
of reference from a friend, teacher, or someone other
than a family member.
- A professional
letter of reference from a therapist, social worker,
teacher, or any other professional with whom you have
contact.
- A one-page letter
stating your reasons for wanting a service dog and
how you feel the dog would benefit you.
Staff, upon receipt of the above, will
review your completed application. Those applicants
we feel qualify for our program based on the application
process will be sent a preliminary interview form to be completed and returned. After receiving this form, we will contact candidates to schedule an interview.
If you are selected for placement, there is a $2200
fee for the dog as well as a $558 fee for the two-week
training course, which is held at our Santa Rosa training
center. |
